Tara Lee, RN, ADN

Session: Collaborative Wound Management with Home Health Nursing for Assisted Living Residents

Bio: Tara Lee, RN, ADN, serves as Vice President of Nursing and Hospice Operations. She is a registered nurse with more than eight years of clinical nursing experience and specialized expertise in the collaborative management of postoperative and wound-care patients in the home health setting. She began her healthcare career at Nationwide Children’s Hospital in 2005 as a State Tested Nursing Assistant and Patient Care Assistant. After earning her Associate Degree in Nursing in 2013, she served as a registered nurse at Nationwide Children’s Hospital from 2013 through 2018, caring for postoperative patients, gastroenterology patients, and individuals receiving care through the Center for Colorectal and Pelvic Reconstruction.

In 2018, Tara transitioned to home health care, where she applied her acute and postoperative nursing experience to improve outcomes for patients at elevated risk for complications and hospitalization. Her work has included developing orthopedic case-management processes to help prevent readmissions, establishing patient-specific communication pathways to reduce hospitalization risk, and creating wound-care case-management teams for assisted living, independent living, and community-based patients.

Tara has also developed processes for wound-supply management and ordering, implemented and supported wound-certified case managers, and managed collaborative relationships with wound centers, primary care providers, surgeons, specialists, and visiting wound-care providers. Her clinical and operational experience supports her subject-matter expertise in wound care, postoperative care transitions, interdisciplinary communication, case management, and strategies to reduce avoidable hospitalizations in home health populations.
