Shannon Little, RN, ADN

Session: Collaborative Wound Management with Home Health Nursing for Assisted Living Residents

Bio: Shannon Little, RN, ADN, serves as Director of Nursing for Home Care at Summit Home Care & Hospice. She is a registered nurse with more than 14 years of nursing experience, all of which has been dedicated to the home health setting. After earning her Associate Degree in Nursing from Central Ohio Technical College in 2011, Shannon began her career as an RN Case Manager in 2012, developing expertise in patient care coordination, chronic disease management, and wound care in the home environment.

Shannon joined Summit Home Care & Hospice in 2015 as an RN Case Manager and advanced into leadership roles, serving as Assistant Director of Nursing for the Assisted Living and Independent Living division before being promoted to Director of Nursing in 2023. Throughout her tenure, she has helped develop clinical processes and care coordination strategies aimed at improving patient outcomes and strengthening collaboration between home health clinicians and facility-based care teams.

A significant focus of Shannon's clinical and leadership experience has been wound care management. Working alongside organizational leadership and interdisciplinary partners, she helped develop wound care workflows, case management processes, and communication pathways that enhanced collaboration between home care staff and Assisted Living and Independent Living communities. She has also supported the implementation of wound care best practices, staff education initiatives, and care coordination strategies for patients with complex wounds and other high-risk conditions. In her current role, Shannon oversees clinical operations, staff development, quality improvement initiatives, and regulatory compliance for home health services. She leads clinical teams caring for a patient population with a high prevalence of wound care needs, bringing both frontline clinical experience and nursing leadership expertise to the management of complex wound care in the home setting.
